Use File Name On Consent Forms

When you add a consent form via horses you will select the form which will have a name such as “Consent for Sedation”. If this box is ticked, then the name of procedure when recording the consent form is called “Consent for Sedation”. If it is not ticked then you have to enter a name before you can save the consent form.
