Adding Treatment Details

Firstly, the only boxes that must be completed are the ones coloured yellow, i.e. Type of Treatment, the date  (will automatically use today's date but can be changed using the calendar button) and the Description.  The rest of the details are optional but however you will get more information out of the system the more you enter in.
