Adding a Consent Form

You can add a consent form from various areas of the system depending on the system itself. However in general it is available from the treatment or history record.

 

From this are you will see a list of consent forms which can be added to by use of the Add button. After clicking Add you will be shown the list of your own consent forms you have created above and you can select one.

 

The form will be loaded and the variables will be replaced by the actual values. So for example today's date replaces %date%, the client forename + surname replaces %client% and so on.

 

You should provide the name of the procedure after which the Signature button becomes available. On clicking this (optional) you will be shown the signature window.

 

Signature

 

This is where a client can use a Tablet light pen for example to provide a signature. A mouse can also be used although better control is available with a light pen. The signature can be cleared if unsuitable or click on OK to save the signature

 

Once OK is pressed the variable %signature% on the consent form is replaced with the image of the signature. This provides a permanently electronically signed consent form.
